This is why we needed to have 4 auto berths from the top leagues. These groups are so much better than in recent years. Liverpool/PSG/Napoli? Barca/Spurs/Inter/PSV Eindhoven? Juve/Man U/Valencia? Atletico/Dortmund/Monaco? Those are all excellent groups! Way better than the two good teams/two dogshit teams format that we’d had for the most part previously.
meh... it doesn't make that much difference since the 4th place team from the top leagues would normally qualify for the group stage anyway through the playoffs. Well, other than you have one additional German team (Dortmund) instead of Dynamo Kyiv or someone like that.
Italy was fourth in the country coefficients used for 2018-2019, so Inter Milan is the club that would have missed out if the fourth place country got two Group Stage spots and one Playoff Round spot. There were also times when two clubs from the top four countries met in the Playoff Round like when Bayer Leverkusen beat Lazio in 2015-2016.
