Or, maybe, "Butt handle"?
Could be worse, I guess.

According to the Spanish wikipedia, he got the name because of his prowess on the field (Debido a su destreza en la cancha, el "Cacha")

And according to this article about the Uruguayan NT nicknames, it is a reference to the handle of a knife or a navaja, which google says can be razor, penknife, pocketknife, etc.
