As far as I know, that's up to the team. Traditionally the Shield is presented to the winning team's fans at the Supporters Summit, but with the new MLS Cup hosting rules affecting the Summit I don't know if that's changed.
Yes, kind of figured that too, though not sure if the supporters / teams have made any arrangements. With S.J. out of the Cup Final now, the chances of the players / coach receiving the trophy themselves become more remote, though the fans could still be representatives by then. As things stand, if the final is L.A. vs Houston (the most likely outcome), the logistics would be simpler: L.A. reps brings the shield to HDC and handover to S.J. reps. Otherwise (e.g. D.C. vs. L.A.), it would be more complicated...
