I have seen it before. Clarence Seedorf had it happen at the end of his career, in Brazil I think. He was being subbed and delayed leaving, so the ref gave him a card. So Seedorf started walking from the far sideline to the bench sideline. The ref told him to leave the field right where he was. Seedorf started walking across the field anyway. Ref warned him, and Seedorf acted ignorantly, as if he could not hear or understand. When ref started jogging over to him, Seedorf started to jog to avoid the 2nd card -- too late -- red card.
Nick Soolsma with TFC got a yellow -> red for a slow walk off the field, back in the day. I think it was him at least. And then he was arrested for public intoxication in Houston after a game and then released. Hell of a legacy at Toronto.

I think the officiating in this league is fine. I just hate how the rules are applied for these type of plays in the box for attacking players in ALL leagues. If you swan dive in front of a defender who is playing the ball, with ZERO intention of actually playing it yourself (as determined by your body position and balance), you are awarded a pk if inside the box almost to a fault. Hell, about 2/3 of Raheem Sterling's penalties are on some form of this play, and the EPL refs buy it every single damn time. It is a joke.
This one bothered me even more because he dove to his left, with the ball ahead of him, to initiate the contact.
If VAR won't overturn the call on the field though, we can expect it to continue. I wish refs would dole out more yellow cards for simulation on dives, whether in the box or not.
It's fair that this is given sometimes in other leagues (England also has a problem with refereeing), but I respectfully disagree on the officiating in the league being fine (and I'm going entirely from this season where I have been watching a lot more MLS). Too great of a range in refereeing quality in MLS for me.
