The order is set in advance. Logistically, this also makes it easier for the referee to track who has already taken kicks.

Quoting myself here, but for clarity, I was curious if this was still the case (as at least it was in tournaments I've been in that have come to penalties). Per the Laws of the Game: Each team is responsible for selecting from the eligible players the order in which they will take the kicks. The referee is not informed of the order. If the referee is not informed of the order, presumably it isn't fixed. I'm not sure if there are more proscriptive procedures for the WC. I do know that if it goes beyond 5 kicks and then through the rest of the order, teams can reshuffle after everyone has taken one kick.

Given that he had already been feeling ill for several days, it seems like there's at least a consistent picture other than foul play. Thw symptoms he was describing earlier in the week sound a lot like little MIs.
