With testing typically done 2 or 3 days before travel (until recently) it's not too surprising. And the number of people submitting fake results would blow your mind. The problem is policy and a lack of will from governments and the public to do something about it. So here we go, thousands more dying because money or some other such BS.

Yes - e.g a common quarantine hotel scenario is test before flight. Then quarantine. Tests day 3 and 11. That is actually better than before, when the first test was usually day 3 after arrival. Even still many don't test positive until day 11.

Well, these folks tested positive when they'd just landed, so... Like I say, something appeared to be going wrong which is presumably why they changed it. Whether this new setup achieves anything we'll see.
I sometimes wonder how the pandemic this year would have turned out if Trump had won *shudder*. Would vaccination rates be any higher? It's undeniable some Republicans are refusing the vaccines for purely political reasons: you just have to look at any news report interviewing them. Fox would surely not be a platform for skepticism of the vaccine the way it is now, if Dear Leader's name was attached to it. But as you say, the denialism was there in 2020 when he was in power and I'm not sure why the existence of vaccines would change that. Guess it's easier to pretend there is not a problem than doing the work to solve it.
Absolutely--the denial, and accompanying obstructionism, would have continued. Contrary to a lot of conventional wisdom ("Mussolini at least got the trains to run on time"), being incompetent at governing is a feature, not a bug, of autocratic regimes.
I encountered lots of them before Jenny McCarthy began to be well-known for her anti-vax views. But she had two big things going for her that other anti-vaxers I was aware of didn't/don't, and they helped her views get more attention.

Remember…there are two steps in testing. Sample collection….and the tests itself. Poor sample collection can affect test accuracy. If the virus/antigen/antibody is present in your body…but evades the collection device….it doesn’t matter how accurate or sensitive the test is.

Y'see, I can absolutely accept that a proportion of the tests aren't accurate, for whatever reason. But I think, when 10% of a large number of passengers arrive and are found to be infected immediately upon arrival, that's not how it's intended to work, presumably.
