No Ching isn't healthy. I don't think he can add another player, but since this is CONCACAF and they could have weird rules, I don't know.
During the last Gold Cup in 2003, each team was allowed to submit a list of eight potential replacements if there were injuries. I've not seen anything like this for 2005, and I can't find anything that indicates that this rule is still in effect. Given that the USSF announced those reserves in '03, and we've not heard anything like this in '05, the rule may have changed......
