Like I said at the beginning, Flamengo shouldn't be grouped with Paraná or even Grêmio for that matter.
The teams I've liked so far: Colo Colo (CHI), Santos (BRA), and Nacional (URU). I haven't seen any of Flamengo's games, but I know they have a lot of points. I would have to watch one of their matches, though. I liked Necaxa when I saw them in their home game vs. Sao Paulo, but they were killed in the return match in Brazil and lost to Audax Italiano this week. Sao Paulo and Boca are another two teams who have good chances in this Libertadores, despite neither of the two actually dominating their respective groups.

Not that I'm aware of.. I checked on Wikipedia from 2000 (when the current format was adapted) till now, and it never happened. Other years had different cases, such as defending champions go directly to the second round, so that's a different strory.
