Re Italy: Pretty amazing, though one could see this coming the past decade or so. All it was gonna take was an unlucky draw and that happened when they got Sweden. Ditto for Netherlands, except even less surprising. 2014 was their aberration - and really aside from 2 good games in that WC they've been utter crap throughout the past 6 years. USA? Probably the most shocking but only because of the region they try to qualify from. Chile? Not all that surprising to see any team not called 'Brasil' or 'Argentina' fail to qualify from that region. Wales? Not surprising at all, especially with Bale injured two-thirds of the time since beginning of qualifying.
