That ranking puts too much emphasis to national championships.

Estudiantes won four Libertadores, that should put them easily into the top 10.
Gremio won four Libertadores, that should put them easily into the top 15.

River won just three Libertadores, they can't be in the top 5.
Colo-Colo played just two Libertadores finals, they can't be in the top 15.
Corinthians played just one Libertadores final, they can't be in the top 20.

I really hope you came from the future, from December 2018 to be more exact.

Grêmio also won a World Title and tied with that GREAT Ajax team of 1995, which was basically the Dutch national team.

Also, in Brazil, the Brazil Cup has a great weight, maybe unlike national Cups in other S.American countries? And Grêmio won five Brazil Cups. So does Cruzeiro.

But Grêmio has one Libertadores more. Plus the intercontinental title.

I gave Colombian league higher rank than say leagues from Paraguay, Chile and Peru (highest points in general also among the best in the world), however the number of domestic league titles in comparison to the rest indeed played a key role in that respect.
I also think that you should consider the national team performances (historically), Brazil and Argentina would be on the front they are the more traditional leagues.
For another hypothetical example, Peru has very low league in South America their best period of the level of performance success at the Copa Libertadores finals came in the 1970s which it was a glorious decade for Peruvian football.
Peru had a decent generations in the 1930s & 1950s before the Copa Libertadores was born this partly is an indicator of national championships quality.
In conclusion, NTs performances to maintain the balance as well, everything taken care of and arranged, so no worries.

Absolutely we have to give a lot of credit to CL performers but not all the credit and yes national league are important too.
Continental competitions is not everything in club football.

And let's not forget Independiente won seven Libertadores that should put them easily into the number one.

Well, I think it's reasonable place for Gremio head as the fifth best Brazilian club of all time, whom above achieved better domestically and internationally combined.

Argentinos Juniors won the Cup in 1985, aside from local tournaments and developing a ton of players, how are they not on this list?

I tried to make a list of the top 50 clubs, but couldn't. South American football is very rich in its history. But... I think Peñarol and Boca are the biggest clubs, sharing first place together.
