That would work for Mexico against USA, but if I wanted to do it for Trinidad and Tobago against Costa Rica, and I couldn't find a list of their head-to-head games, I would have to spend time getting the information from ELO's list of all the games played by Trinidad and Tobago or Costa Rica (I could pick either one to look at). If I wanted the head-to-head results between each pair of the 12 teams in the WCQ Semifinals, that would be 66 pairs of teams. If you know of a website with a list of head-to-head games between any two members of FIFA who have ever played each other, please tell me. I don't mean to be rude. I'm just saying that the information I want can be time-consuming to gather.

Of the 25 teams not listed above, 8 moved at least 20 spots: Haiti was down 26 to 100th Puerto Rico was up 46 to 112th El Salvador was down 26 to 125th Curacao was up 20 to 124th Belize was down 50 to 164th Suriname was up 20 to 170th St. Lucia was down 26 to 171st Bermuda was down 21 to 188th It is the first time Puerto Rico was ever ranked ahead of El Salvador.
Puerto Rico almost doubled their FIFA Ranking points! They gained 165 points to 332. France, Italy, Wales, Croatia, Poland, and Guinea-Bissau were the six countries to gain more points than Puerto Rico this month. Puerto Rico had the second most points gained by a country not in UEFA.
PR win also to Grenada on penaltis... that was an espectacular game, overcome a 3-0 at 38min, tie and win on PK's.

Costa Rica (up 6), Guatemala (up 8), and Curacao (up 3) are the only three CONCACAF countries to move up at least 3 spots. Trinidad and Tobago (down 10), Panama (down 18), Jamaica (down 16), El Salvador (down 12), and Cuba (down 30) fell at least 10 spots. Cuba fell the most points and the most spots in the world. El Salvador is 137th, which is their worst ranking since they were 141st in November 2007.

With Curacao being tied for the second most spots gained in the world, http://www.football-rankings.info/2016/10/fifa-ranking-october-2016-final-preview.html says the Top 9 in CONCACAF in the October 2016 FIFA Rankings will have five Caribbean teams. The FIFA Rankings will be released on Thursday 20 October. I checked the FIFA Rankings back to the 200th rankings from March 2011, and there have never been 5 Caribbean teams in the Top 9 in CONCACAF. Later I will check farther back.

I checked all 267 FIFA Rankings, and this is the first time the Caribbean had 5 of the Top 9 in CONCACAF.
In Thursday's FIFA Rankings, Curacao will become the top team in the Caribbean. The Top 5 in the Caribbean will be very close: Curacao: 467 points Jamaica: 461 points (UNCAF team Honduras will also have 461) Trinidad and Tobago: 454 points St. Kitts and Nevis: 443 points Antigua and Barbuda: 433 points Costa Rica will pass Mexico. I'll post more when the rankings are released.

The May 2017 FIFA Rankings didn't have many changes. The only team in the Top 10 in CONCACAF in April 2017 to move was Jamaica, who was down 1 to 74th. 24 out of 35 teams didn't move, 4 moved up 1, 4 moved down 1, St. Lucia was down 2, and Grenada (now 169th) and St. Vincent and the Grenadines (now 170th) were up 7 spots.
