What determines what kit a side wears for a match?

I was having a discussion with some friends and we were wondering what the rules are on kit determination. Who or what decides what side wears what kit to a match?

Usually both teams wear their home kits, if they have the same color, the away team has to wear their away jersey, or only change the shorts or socks. The away team has to wear a kit that doesn't match the home team's kit. The home team can always wear any of their jerseys, usually they have a fixed home kit but they don't have to.
There is also one 'home' and one 'away' team when they play on neutral ground, like e.g in a WC. Who's home and who's away is drawn. Many teams have also 'special' jerseys, which they only wear on special occassions, like e.g in Champions League, season opening or rivalry games.
