Try typing in any info for ticket holder #4. The info goes into the match fields of ticket holder #5. I had to hand write that info.

I also noticed a bug on their form. If you enter a category for the third match for the ticket holder #1, that same number shows up in the category field for ticket holder #4. I hope that doesn't invalidate my application. :)
Somehow I saw that & just deleted it for th#4; in any event I'm not trying to fax that page!

What a joke. The PDF form doesn't work correctly, and the idiots that run USSF only setup ONE STINKING FAX LINE!!!!!

I"ll keep trying until the end of the day, then I'll FedEx it tomorrow and hope for the best.

Just a FYI--someone in the Fed told me this morning that they were not allowed, by FIFA, to set something up via email/the net.

No reason to doubt what they said, in the end, at least in my opinion.
