I think the discussion begins and ends with Patrick Ewing in Exorcist 3.
But am willing to hear other discussions.

Starsky and Hutch did it all wrong.
I like how Lou Ferrigno was inserted into the Hulk.
I'd guess many of these old TV shows made into movies needs a cameo from the original star, no?

My favorite is The Boss in High Fidelity.
There's a Billy Crystal/Robin Williams movie where they both think they are the father of the same boy where Mel Gibson shows up uncredited as very pierced tatoo artist.

Erin Brockovich in "Erin Brockovich"
Alfred Hitchcock in "Lifeboat"
Marshall McLuhan in --is it "Annie Hall?"
Patrick McNee in "The Avengers"
Not really a cameo, but maybe the best preformance in a tiny part ever? Orson Welles in "A Man For All Seasons."

Yes it is. And that's a winner.
This one's a bit boring but kinda funny:
Quentin Tarantino's producer, Lawrence Bender, has a cameo at the end of "Pulp Fiction" as one of the customers in the restaurant being held up.
He's roughed up and thrown to the ground. In the credits he's listed as "Long Haired Yuppie Scum." I think maybe Roth refers to him as such.

Wasn't Alfred Hitchcock in all of his movies?
I think Quentin Tarantino does this, too.

I'll add some silly cameo's that got a laugh for me:
- Charlie Sheen in 'Loaded Weapon 1'.
- Charlton Heston in 'Wayne's World 2'.

In the remake of Cape Fear both actors who stared in the original movie had cameos Robert Mitchum and Gregory Peck.
Incidently the original in my oppionion was better then the remake less shocking but you had to be effected by Mitchums performance.

Yes Hitch was in all his movies. I think Scorcese is in all his as well.

Yes, but Hitch wasn't exactly "in" Lifeboat. I believe a drawing in
profile of his body shape in a newspaper was. I understand he
wanted initially to be a body floating in the water but thought better
of that.

In the movie "after hours", Martin Scorcese is one of the dudes at Club Berlin holding a spotlight and shining it around.
I'm also a fan of Wes Anderson's cameo in The Royal Tennenbaums, playing the Tennis announcers at Richie's meltdown game.
I dunno if those count, since thoe are both directors taht chip in and do small things in their own movies...but whatever.

A lot of people don't know this, but one of the people who works in Oskar Schindler's factory in Schindler's List is none other than Ron Jeremy.

Kevin McCarthy starred in the original Invasion of the Body Snatchers, which ended with him exposing the alien invasion by shouting "they're here, they're here!" and the movie ended on a hopeful note.
Near the beginning of the Donald Sutherland version of Invasion of the Body Snatchers, Sutherland's cab hits and kills a crazy guy who's run into the street shouting "they're here, they're here!" The crazy guy is Kevin McCarthy, of course.

Sports movies and slapstick comedies seem to get the most cameos, but my favorite is Samuel L. Jackson in the final scene of "Out of Sight".

It was #3.
It was Tom Cruise as Austin Powers, Gwynneth Paltrow as Felicity Shagwell, Kevin Spacey as Dr. Evil, Danny De Vito as Mini Me and Steven Spielberg was the director. And Katie Couric was a prison guard in that movie, apparently.
