The race motorcycle ahead of Phinney in the stage 21 time trial missed a turn on the course and led the American off-course. Phinney was forced to turn around and return to the course.
The Spaniard needs a miracle and Hesjedal is leading the Giro because the gap between them is 44 seconds.

Funny thing is that many cycling pundits in Belgium had been giving De Gendt lots of grief for planning his wedding during the Tour. Yet if he hadn't done that, he likely wouldn't have ridden the Giro and this podium finish never would've happened.
Oh no, they aren't still complaining now! But they were before the did so well in the Giro. Funny how things turn out sometimes.
