IIRC, Jesse was eventually told by the doctors (or maybe by Brock's mother) that it was Lily of the Valley. He actually told Walter this when they were on the rooftop right after Gus got 'sploded.
It's not that hard to believe. He has had the suspicion the entire time and after telling Walter to his face that he knows he has been bullshitting him, you know his suspcions about Walter poisoned Brock were still there. He didnt need much to put things together. Have you ever had a situation like that? Something as simple as that helping you solve a big puzzle. I have and it can happen that easily, especially when you have had time to sit and dwell on things that have happened. Jesse has been sitting in his house, depressed doing nothing but thinking for weeks at this point. He likely has ran every scenario and situation through his head multiple times out of guilt alone. I thought it was brilliant writing.
