    Specifically to all this...

Ramires is suspended for his 3rd caution.

Ivanovic is suspended for his 3rd caution.

Meireles is suspended for his 5th caution (having presumably already been suspended for the second leg against Napoli after his 3rd caution... but not again suspended after the 4th caution).

Lampard is not suspended as it is only his 2nd caution.

It's in that first sentence. Specifically, the first four words...
Once the group stage begins, the bookings count never resets.
A player can get booked on group stage Matchday 1 and group stage Matchday 2, then receive a third in the semifinal second leg and he would miss the final. :(

Last night after the penalty was awarded for Drogba's foul on Fabregas, Ivanovic was booked for dissent before the penalty was taken. His third yellow of the tournament = suspension.

His third booking was in the Napoli first leg, thus he missed out on the Napoli second leg and returned for Benfica.

    EDIT:
MassRef covered it all while I was typing all that.

I'm sorry, but I disagree. Chec was wasting time. If I recall correctly, the ball went out for a goal kick and went behind the net. The ball boy tossed Chec a ball, which he caught. Instead of restarting with the ball he was carrying, Chec went behind the net to get the other ball. He was absolutely delaying the restart of play. A good call, and one that actually had an effect on the game, because Chec wasn't about to risk getting a second yellow in the thirty minutes that remained.

I hate when referees repeatedly scream at goalies throughout the second half to put the ball back in play quickly, with no effect, and then wait until the last few minutes of the game to give the caution. At that point, the card has little effect, because the goalie has already accomplished his object: Repeatedly delaying the restart over the course of the second half and wasting time.

    Thanks.
I find it all a bit silly.
You get a yellow in the second leg of the finals:
If it's your third, you miss the final.
If it's your fourth, you're good to go.

And people wonder why this leads to intentional cautions.

I understand the policies of this forum so I will only be posting once.

Alberto, you are simply wrong. The video CLEARLY shows Sanchez is indeed backing in to Terry and that he embellished the foul.

Terry deserved red, but Alberto is, as usual, being intentionally obtuse.

Sanchez was backing up. I wouldn't necessarily say he was backing into Terry. What I would say is that Terry took a step forward and deliberately brought his knee up into Sanchez. Even on the long shot (as opposed to the closeup), it's an easy VC sendoff if you see it. Good on the AR (or was it actually an AAR) who saw it.

You are entitled to your opinion, but Terry is a player that routinely loses his mind and commits misconduct that gets his team in trouble. I will readily admit I loathe him as a player and as a human being. He fall far short on both counts in my book. He also routinely gives tremendous amounts of crap to officials. Yet another reason I can't stand him.

Okay, so we agree Terry committed an offense worthy of a send off. Sanchez walked back, but Terry buried his knee into his tailbone. Having fallen on my tailbone, I can attest it hurts like hell. Could Sanchez have embellished it? Perhaps, but players today consistently make a meal of any contact.

Perfectly said, in my opinion. This is the mentality that referees need to employ when considering a sanction for DOGSO. Consider all the criteria, then make a judgment on whether or not the foul denied an obvious goal-scoring opportunity. It will lead to more common sense and, ultimately, more consistency in the law's application.
